Alcohol withdrawal syndrome

Risk

  1. Male > 2 SD/day หรือ > 14 SD/wk
  2. Female > 1 SD/day หรือ >7 SD/wk

[ SD = Standard drink = alcohol 10 gm]

Alcohol (gm) = %/100 * volume (ml) * 0.789

Criteria

Heavy drinking + หยุดทันที + AWS

  1. Tremor (6 hr) + autonomic hyperactivity
  2. Hallucination (12 hr) ; auditory หูแว่ว
  3. Seizure (24 hr) ; GTC, recurrent 25%
  4. Delirium Tremens (72 hr) ; fluctuation of disorientation 2-3 วัน
    • เคยเป็น, ชัก (>1/3), อายุมาก, >300 mg%

Approach

  1. R/O other causes
  2. ประเมิน Severity : F/U, caregiver, suicidal, AWS score
    • Mild – mod : OPD
    • Severe : Admit IPD

Management

  1. BDZ for calm down
    • OPD:
      D(10) po q 6 hr c prn (day 1)
      D(5) po q 6 hr c prn (day 2-5)
    • IPD
      Valium 10 mg IV q 15-30 min until contol, then q 2-4 hr (MAX 300 mg/day)
      (Total dose/day แบ่งให้ 4 เวลา จนอาการ stable > tape dose 25% q 2 day)
      หยุดยา 7-10 วัน
    • Hallucination: Haloperidol 5 mg IM q 6 hr c prn
    • Seizure: Sodium valproate loading 20 MKd แบ่ง bid q 6-8 hr then q 12 hr x 4 day
  2. Symptomatic
    • Metoclopramide 10 mg PO or IM q 8 hr c prn for N/V
    • Para
  3. Supportive
    • Thiamine (B1) for WK prevention
      • Vit B1 (100) IM OD * 3 days
      • Vit B1 (100) 1×3 po pc * 3 days
    • MTV 1×3 po pc
    • IV or Oral hydration
    • Correct electrolytes
